- Current CPR certification
- Strong communication and interpersonal skills
- Wound vac (KCI) experience and IV infusion experience are bonus
- Bilingual a plus, Spanish highly preferred

We are a home health agency on the north side looking for a full-time Clinical Care Coordinator with excellent computer and communication skills. With oversight from the Agency Supervisor and Nursing Supervisor, this person assists the day-to-day nursing and therapy activities and assumes the accountability for quality services and nursing care. You will also assist in maintaining a physical, social, and psychological environment which will be conducive to the best interest and welfare of the client in conjunction with our core values.
Essential Functions and Responsibilities:
- Assists Supervisors with auditing and maintaining clinical policies and procedures as necessary to respond to changes in State and Federal regulations and/or to update on additional programs and initiatives (including but not limited to infection control, skin assessments, safety and client rights.)
- Assists Supervisors in maintaining records of all clients with infections, collects data, and prepares reports.
- Assists Supervisors with in-services to teach all employees information pertinent to infection control and other pertinent disciplines. Provides instruction to staff concerning quality of care and quality of life issues, and customer service and positive image.
- Assists Supervisors in coordinating the quality assessment and assurance committee, including the areas of infection control, pharmacy, and utilization review. Periodically audits, reviews and provides input on policies and procedures in conjunction with committee members.
- Assists Supervisors in reviewing unresolved issues/concerns and any other areas that remain out of compliance from prior meetings to develop a timeframe for correction. Discusses and promulgates issues/ concerns within the agency and collaborates with other clinicians to maintain compliance.
- Assists Supervisors in establishing and maintaining tracking systems for data collection and recommendations related to quality assurance.
- Perform duties of field staff nurse whenever and wherever required including but not limited to completing health assessments, reviewing, implementing and evaluating client’s medication, treatment, wound care, Foley care, and other skilled nursing services.
- Actively participates in QAPI (Quality Assurance Process Improvement) program, taking a proactive approach to quality care and services on a continuous basis with engagement of clients, family members and staff.
- Performs other duties as assigned by Supervisors.
- Monitors delivery of care and documentation, identifying progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care providers; interacting with involved departments to negotiate and expedite scheduling and completion of tests, procedures, and consults; reporting personnel and performance issues to the Supervisors; and maintaining ongoing communication with Supervisors and staff regarding variances from the care plan or transfer/discharge plan.
Contributes to team effort by accomplishing related results as needed.
Performs other duties as assigned by Supervisors.
Qualifications, Education and Experience
- High School Diploma or equivalency (required).
- Valid LPN or RN license in the state of Illinois
- 1 year of practical nursing experience in a clinical setting or long term care facility (preferred).
- At least 1 year of home health experience with knowledge of home health process and services. In-office home health experience preferred.
- Possesses the ability to make independent decisions when circumstances warrant such action

Premier Point provides 5-star home care medical services. We are growing and we need the services of a per diem Home Health Aide for visits in the north and northwest suburbs of Chicago. This person must be a Certified Nursing Assistant (CNA) for home health aide visits. The home health aide will work under the supervision of a registered nurse or therapist to perform various personal care services as necessary to meet the patient's needs. This person will be responsible for observing patients, reporting these observations and documenting observations and care performed. The HHA will be assigned in a manner that promotes quality, continuity and safety of a patient's care
Responsibilities include, but are not limited to:
- Providing hands-on personal care including: baths, back rubs, oral hyiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, and keeping the patient's living area clean and orderly, as appropriate
- Planning and preparing nutritious meals
- Assisting in feeding the patient, if necessary
- Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered
- Assisting in ambulation and exercise according to their plan of care
- Performing range of motion and other simple procedures as an extensional therapy service as ordered
- Doing patient's laundry, as appropriate
- Meeting safety needs of patients and using equipment safely and properly
- Reporting on patient's condition and significant changes to the assigned nurse
- Adhering to Premier Point Home Health's documentation and care procedures and standards of personal and professional conduct
Qualifications and Skills
