Home Health Aide Services

 

Whether you or a loved one are recovering from an injury, coping with illness, or finding it difficult to be as active, Premier Point Home Health can help to maintain dignity and freedom in a safe and healing home care environment.

Our Home Health Aides provide a variety of non-medical services that help clients manage daily living activities, and relieve the stress and worry of family members struggling to provide care while juggling the demands of work, school, and all of life's challenges. Our Home Health services include:

  • Elder in-home private duty services including assistance with bathing, shaving, toileting, dressing, hair care and oral care
  • Homemaker companionship including light housekeeping, meal preparation, and feeding assistance
  • Nutritional assistance including encouraging or restricting fluids and preparing meals according to a defined dietary plan
  • 24-hour caregiver/sitter assistance

We also offer skilled nursing care services that could include:

  • Checking vitals and coordinating follow up medical care if irregularities are identified
  • Protime testing – Coumadin
  • Bowel training
  • Catheter or ostomy care and emptying drainage bag, urinal, or beside commode
  • Wound care
  • Geriatric case management
  • Administrating medications, in-home IV therapy, and other pain management treatment
  • Coordinating physical, occupational, and speech therapy, if needed

Nursing care services and home health aide support may be covered 100% under the patient’s Medicare or private insurance plans.  We also offer competitive payment arrangements for direct pay patients.

Therapy Services

 

Many patients have complex health care needs and are struggling with decreased functionality due to stroke, neurological diseases, amputation and other disabilities, or aging. Dependent on the individual patient need, family or caregivers oftentimes want to know whether the person can function independently and safely.

Coordinating with the referring physician, Premier Point's on-staff physical, occupational, and speech therapists will discuss with the patient the severity of the pain, injury, or disability, assess current functionality, and develop a therapy regimen aimed at improving, adapting or restoring functions which have been impaired or permanently lost with the goal of achieving measurable improvements. As part of the care plan development process, our therapists work with patients to establish realistic goals, and determine how many therapy visits it may take to reach desired outcomes.

Premier Point's Occupational therapists are experts in helping patients to regain or utilize adaptive techniques to re-master skills needed for daily living and gainful employment. Therapy provided includes bowel/bladder management, bathing and personal hygiene needs; identifying and learning to use medical equipment and assistive devices; and community mobility and job performance.

Our on-staff Speech Language pathologists help patients overcome or adapt to disorders resulting from a variety of causes including brain injuries, Parkinson's disease and cerebral palsy, hearing loss or emotional issues. We also work with Infants with injuries due to complications at birth, feeding and swallowing difficulties, including dysphagia; and social communication and cognitive-communication skills training including attention, memory, and problem solving.

Premier Point's Physical therapists develop customized care plans based upon the specific condition presented. Plans offer a broad array of therapy techniques including directed and monitored endurance exercises; specific repeated movements to reduce symptoms; strengthening and stabilization exercises; and appropriate education regarding the patient's role in managing their condition.

We also offer medical social worker consultation services for patients who need assistance coordinating with the community to secure living and financial management aide, or referrals for crisis intervention and long-term planning.

Medical Social Worker Services

 

Medical conditions affect far more than the body. Acute, chronic and terminal conditions can result in emotional, financial, and social needs that sometimes overwhelm patients and impact the treatment process. 

 

Our licensed medical social workers help patients and families manage nonmedical support needs such as financial and legal assistance with the goal of maximizing stability.  Services can include:  

  • Performing a psychosocial assessment to define needs, and working with the patient and family members to set realistic goals and develop a therapeutic plan
  • Identifying and utilizing resources within the community that support the patient’s plan of care including connecting the patient with specialized providers to procure needed services such as custom medical equipment or adaptive tools, home modifications, and transportation services
  • Providing patient counseling or referrals to support psychosocial needs
  • Interpreting attitudes and patterns of behavior that may negatively impact treatment goals and helping the patient to identify alternate options, or performing crisis intervention where necessary
  • Providing support, counseling and guidance to family members and caregivers to ensure the integrity of the plan of care is maintained 

 

 

Registered Dietician Services

 

Our registered dieticians use their nutrition expertise to counsel patients undergoing treatment for a variety of medical conditions including diabetes, heart disease, high cholesterol, high blood pressure, and gastrointestinal disorders on how to make unique, positive lifestyle changes that support their medical care plans.  Services offered include:

    • Evaluating patient’s food and fluid intake versus their nutritional needs as defined in the care plan and provide recommendations
    • Developing customized dietary programs that support treatment goals including weight management
    • Providing counseling to patient’s family and caregivers to ensure understanding of the critical interaction between diet and treatment goals, and obtaining buy in and support of the dietary program
    • Reviewing prescribed medications and vitamin supplements for potential dietary conflicts and make recommendations for food restrictions or notify the physician for further review
    • Educating the patient on dietary progress to goals based upon relevant lab results
